Declaration / Undertaking by the Applicant
  1. I hereby declare that all the information furnished by me in the application form is true and correct to the best of my knowledge and belief.
  2. I undertake that I shall produce all my original documents for verification immediately whenever demanded by the concerned authority. Failing which, the University shall be free to cancel my admission.
  3. I declare that I shall mandatorily attend the University Orientation (Induction) Programme on the scheduled date. In case of non-attendance, I shall accept the penalty as decided by the University authorities.
  4. I further declare that I am not suffering from any serious or contagious disease or from any mental/psychological disorder.
  5. I undertake to inform Oriental University from time to time about any change in the information provided in the application form or other documents, such as address, phone number, etc.
  6. I undertake to submit the original Transfer Certificate (TC) and Migration Certificate within a period of two months from the date of admission (if not available at the time of admission). Failure to do so will result in my semester examination form not being forwarded.
  7. I undertake to strictly follow the dress code and uniform prescribed by the University.
  8. I undertake to appear in University examinations honestly and shall not use any unfair means. In case of violation, I shall accept the penalty imposed by the University authorities.
  9. I shall regularly check the notices displayed on departmental, library, and office notice boards, as well as the University website and other digital platforms, and shall be personally responsible for all student-related information published there.
  10. I am aware that the University campus is under CCTV surveillance and that I may be identified in case of any misconduct or illegal activity. In such cases, I shall accept the penalty imposed by the University authorities.
  11. If I am found violating any disciplinary rules, I shall be liable to face disciplinary action as decided by the University.
  12. I further declare that if at any stage I am found ineligible or if any information provided by me is found to be false, my admission may be cancelled.
  13. I declare that during the admission process, I have been informed about scholarships, and any scholarship amount received by me shall be deposited with the University as per the prescribed rules.
  14. I further declare that any additional information related to scholarships shall be obtained from the Scholarship Department.
  15. I declare that in case of provisional admission, I shall submit all required documents as per University norms. Failing this, the University shall be free to cancel my admission.
  16. After admission, for any further information, I shall obtain details from the Head of the Department / TG (Teacher Guardian) / Dean / concerned authority of the department in which I am admitted.
  17. The fee submission dates prescribed by the University for each academic year shall be as follows: Odd Semester: 31st July (Last Date), Even Semester: 31st January (Last Date).
  18. I undertake to submit my fees and all mandatory certificates to the University within the stipulated period provided by me from the date of admission. Failure to do so will result in my semester examination form not being forwarded.
  19. I undertake to abide by all the rules and regulations of Oriental University relating to admission, attendance (minimum 75% mandatory), discipline, code of conduct, and any amendments made from time to time. I acknowledge that Oriental University has full authority to take disciplinary action against me in case of any violation.
  20. I declare that once admission is taken in a particular course/programme, no change in the course/programme shall be permitted.
  21. I have been informed by the admission counselor during the admission process about all applicable late fees, including but not limited to Tuition Fee, TC & Migration, Examination, Enrollment, etc.
  22. I undertake that all documents shall be submitted to Oriental University by the above-mentioned date. In case of non-submission of documents, Oriental University shall be free to cancel my admission.
  23. I have received basic information regarding hostel, mess, and bus facilities from the admission counselor. For further details, I shall contact the Hostel Warden and Transport In-charge.
  24. In case I cancel my admission for any reason, I have been informed by the admission counselor about the admission cancellation charges and applicable dates as prescribed by Oriental University.

मेरे द्वारा निम्लिखित घोषणा/शपथ करता/करती हूँ
  1. मैं यह घोषणा करता/करती हूँ कि आवेदन पत्र में दी गई समस्त जानकारी मेरी सर्वोत्तम जानकारी और विश्वास के अनुसार पूर्णतः सत्य है।
  2. मैं यह शपथ करता/करती हूँ कि संबंधित प्राधिकरण द्वारा माँगे जाने पर मैं अपने सभी मूल दस्तावेज सत्यापन हेतु तत्काल प्रस्तुत करूँगा/करूँगी। अन्यथा विश्वविद्याल पात्र है, मेरा दाखिला निरस्त करने के लिए !
  3. मैं यह घोषणा करता/करती हूँ ,की में विश्वविद्यालय के ओरिएंटेशन (दीक्षारम्भ) समारोह में नियत तिथि पर सम्मिलित होऊँगा/होऊँगी ! ऐसा नहीं किये जाने जाने पर विश्वविद्यालय प्राधिकरण द्वारा निर्धारित दंड स्वीकार होगा !
  4. मैं यह भी घोषणा करता/करती हूँ कि मैं किसी भी गंभीर, संक्रामक रोग या किसी मानसिक/मनोवैज्ञानिक विकार से ग्रसित नहीं हूँ।
  5.  मैं यह शपथ करता/करती हूँ कि आवेदन पत्र या अन्य दस्तावेजों में दी गई जानकारी, जैसे पता या फोन नंबर आदि में किसी भी प्रकार के परिवर्तन की सूचना समय-समय पर ओरिएंटल यूनिवर्सिटी को दूँगा/दूँगी।
  6. मैं प्रवेश की तिथि से दो माह की अवधि के भीतर (यदि प्रवेश के समय उपलब्ध न हो) मूल स्थानांतरण प्रमाण पत्र (TC) एवं माइग्रेशन जमा कर दूँगा/दूँगी। ऐसा न करने पर मेरा सेमेस्टर परीक्षा फॉर्म अग्रेषित नहीं किया जाएगा।
  7. मैं विश्वविद्यालय द्वारा निर्धारित ड्रेस कोड एवं यूनिफॉर्म का अनिवार्य रूप से पालन करूँगा/करूँगी।
  8. मैं विश्वविद्यालय परीक्षाओं में भी ईमानदारी से सम्मिलित होऊँगा/होऊँगी और किसी भी प्रकार के अनुचित साधनों का प्रयोग नहीं करूँगा/करूँगी। उल्लंघन की स्थिति में विश्वविद्यालय प्राधिकरण द्वारा निर्धारित दंड स्वीकार होगा।
  9. मैं विभागीय, पुस्तकालय एवं कार्यालय के सूचना पटों, वेबसाइट तथा अन्य डिजिटल प्लेटफॉर्म पर जारी सूचनाओं को नियमित रूप से देखूँगा/देखूँगी और वहाँ प्रदर्शित छात्र-संबंधी जानकारी के लिए स्वयं जिम्मेदार रहूँगा/रहूँगी।
  10. मैं विश्वविद्यालय में स्थापित सीसीटीवी कैमरा प्रणाली से अवगत हूँ और किसी भी दुर्व्यवहार या अवैध गतिविधि में संलिप्त पाए जाने पर मुझे पहचाना जा सकता है। ऐसी स्थिति में विश्वविद्यालय प्राधिकरण द्वारा निर्धारित दंड स्वीकार होगा।
  11. यदि मैं किसी भी अनुशासनात्मक नियम का उल्लंघन करता/करती पाया गया/पाई गई, तो विश्वविद्यालय द्वारा निर्धारित दंड का पात्र होऊँगा/होऊँगी।
  12. मैं यह भी घोषणा करता/करती हूँ कि यदि किसी भी स्तर पर मैं अयोग्य पाया गया/पाई गई अथवा मेरे द्वारा दी गई जानकारी असत्य पाई गई, तो मेरा प्रवेश रद्द किया जा सकता है।
  13. मैं यह घोषणा करता/करती हूँ , की मेरे एडमिशन प्रक्रिया के दौरान मुझे स्कालरशिप की जानकारी दी गयी है ! और मुझे जो भी स्कालरशिप प्राप्त होगी, वो में विश्वविद्यालय में जमा करूँगा/करूँगी !
  14. मैं यह घोषणा करता/करती हूँ, मुझे एडमिशन प्रक्रिया के दौरान ये बताया गया है,की स्कालरशिप से संबंधित अन्य कोई भी जानकारी स्कालरशिप विभाग से प्राप्त होगी !
  15. मैं यह घोषणा करता/करती हूँ, की मेरे द्वारा अस्थायी प्रवेश (provisional admission) लेने पर मुझे विश्वविद्यालय को समस्त कागज़ात जो की विश्वविद्यालय के मापदंडों के अनुरूप हो , मुझे प्रस्तुत करने होंगे , अन्यथा मेरा प्रवेश निरस्त करने के लिए विश्वविद्यालय स्वतंत्र है !
  16. विश्वविद्यालय में प्रवेश के पश्चात, किसी भी अन्य जानकारी के लिए मुझे उस विभाग के विभागाध्यक्ष / TG (शिक्षक अभिभावक) / डीन / संबंधित प्राधिकारी से सूचना प्राप्त होगी, जिसमें मेरा दाखिला हुआ है।
  17. प्रत्येक शैक्षणिक वर्ष में विश्वविद्यालय द्वारा निर्धारित तिथियाँ, जिन पर शुल्क (फीस) जमा करना अनिवार्य होगा, निम्नानुसार रहेंगी—

               ●  विषम सेमेस्टर (Odd Semester): प्रत्येक वर्ष 31 जुलाई (अंतिम तिथि)

               ●  सम सेमेस्टर (Even Semester): प्रत्येक वर्ष 31 जनवरी (अंतिम तिथि)

  1. मैं प्रवेश की तिथि से मेरे द्वारा दी गयी अवधि के भीतर, मेरी फीस एवं समस्त अनिवार्य प्रमाण-पत्र विश्वविद्यालय को प्रस्तुत करूँगा/करूँगी! ऐसा न करने पर मेरा सेमेस्टर परीक्षा फॉर्म अग्रेषित नहीं किया जाएगा।
  2. मैं ओरिएंटल यूनिवर्सिटी के प्रवेश, उपस्थिति जो की कम से कम 75% अनिवार्य है, अनुशासन आदि से संबंधित सभी नियमों एवं   विनियमों तथा समय-समय पर लागू आचार संहिता एवं उसमें किए गए संशोधनों/परिवर्तनों का पालन करने का वचन देता/देती हूँ। मैं यह स्वीकार करता/करती हूँ कि इनके उल्लंघन की स्थिति में ओरिएंटल यूनिवर्सिटी को मेरे विरुद्ध दंडात्मक कार्रवाई करने का पूर्ण अधिकार है।
  3. मैं यह घोषणा करता/करती हूँ, की मेरे द्वारा एडमिशन जिस कोर्स/प्रोग्राम में लिया है उसके पश्चात इसमें किसी भी प्रकार का बदलाव नहीं होगा !  
  4. मुझको ओरिएण्टल विश्वविद्यालय द्वारा सभी प्रकार के विलम्ब शुल्क (Late Fees) से, काउंसलर ने एडमिशन प्रक्रिया के दौरान अवगत करवाया है, जिसमे प्रमुख रुप से (Tuition Fee, TC & Migration, Exam, Enrollment,Etc.) Late Fees शामिल है! 
  5. मेरे द्वारा दी गयी उपरोक्त दिनांक को सभी दस्तावेज़ ओरिएण्टल विश्वविद्यालय में जमा कर दिए जायेंगे। दस्तावेज़ जमा नहीं करने पर ओरिएण्टल विश्वविद्यालय स्वतंत्र है, मुझका दाखिला निरस्त करने के लिए! 
  6. एडमिशन परामर्शदाता द्वारा मुझे हॉस्टल, मेस एवं बस सुविधा के विषय में बुनियादी जानकारी प्राप्त हुई है, मुझआवेदक को और कोई अन्य जानकारी प्राप्त करनी है तो मुझे हॉस्टल वार्डन और ट्रांसपोर्ट इंचार्ज से संपर्क करना होगा! 
  7. किसी कारण यदि में अपना एडमिशन निरस्त (Cancel) करवाता हुं, तो मुझे ओरिएण्टल विश्वविद्यालय द्वारा एडमिशन कैंसलेशन शुल्क एवं तारीखों  के बारे में, एडमिशन परामर्शदाता ने मुझेअवगत करवाया है।
